Capalaba Medical Centre is one of the largest general practice providers in Queensland. It is home to a multitude of doctors with many diverse skills. Many of the doctors have been practicing for some decades which brings immense experience to a complicated case. The centre manages most complex medical cases without the need for expensive specialist referrals. Chronic disease in particular requires multidisciplinary skills particularly when considering more than one disease in the same patient. At present, Medicare provides limited funding for allied health professionals (podiatrist, physiotherapist, dieticians, exercise physiologists etc) to assist in the complex management required for Chronic Disease. This can be done via a General Practice Enhanced Primary Care Chronic Disease Management plan. The Practitioners at CMC are frequently employing these cost effective plans to achieve the best outcomes for our more complex cases.
